Note to faculty member making the referral: Any information you provide may be shared with the student, who has legal access to educational records at Boise State. Observations should be objectively stated.

Checklist/Classroom Observations

Check any statements that describe the student.

Student lacks essential reading skills

Student lacks essential writing skills

Student lacks essential math skills

Student is not completing assignments/homework

Student has missed quizzes, exams, etc

Student is not attending class

Student's work is below expectations for this course

Student demonstrates a lack of organization in work

Student is continually late to class

Student is attending class, but not making satisfactory progress

Student is not engaged or participating in class discussions

Student is not performing well on quizzes/tests

Student exhibits disruptive classroom behavior

Student struggles to maintain good grooming/hygiene

Student has demonstrated drastic changes in grooming/hygiene

Student responds inappropriately to instructor
